What is an Assisted Living Facility?

Assisted living is a general term that is used across the United States to describe a residence for seniors where assistance is provided, as needed, with activities of daily living, such as bathing, dressing, grooming, ordering and taking medications, and making doctors’ appointments. Additionally, meals, snacks, housekeeping and laundry services are provided. The goal is to provide each person living in these residences with considerate and respectful care, and to promote their dignity, independence, privacy and safety in a comfortable, residential setting.

What is an ALP or Assisted Living Program?

The Assisted Living Program (ALP) is an Adult Home or Enriched Housing Program that is certified by the New York State Department of Health to provide Medicaid-funded home care services. The ALP is an alternative to nursing home that enables nursing home eligible residents to receive home health care services in the Adult Home or Enriched Housing Program, delaying or preventing the need for admission to a nursing home. Care services are provided by an on-site licensed home care agency operated by the Adult Home or Enriched Housing Program and a community certified home health agency (CHHA) under contract.

Are the facilities licensed? How is assisted Living Regulated?

Yes, all Gefen Senior Care’s assisted living facilities are licensed by the New York State Department of Health, and adhere to the New York State Department of Health (NYSDOH) adult care facility regulations.

Is it 24 hour staffed?

Yes, all facilities are staffed 24 hours a day, 7 days a week including weekends and holidays.

Am I free to come in and go out of facility as I please?

Yes, all residents are free to come in and out of the facility, as they please.

Do I get assistance taking medication?

Yes, our staff is highly trained to assist all our residents with their self-administration of medication needs.

Do I need to bring furniture?

No, you do not need to bring your own furniture. All facilities provide our residents with high quality and comfortably designed furniture in each room. However, if you would like to bring in your own furniture you can do that. Arrangements need to be made with the administration.

Are there private rooms?

Yes, private rooms are available in all facilities. Please inquire about availability and pricing.

Are there doctors on site?

The facility can assist the resident in arranging for a variety of medical and mental health services that are often provided on-site or in the community depending on the individualized need of each resident. These include primary medical services, psychiatry, podiatry, dentist, optometry, and various other medical specialists.

What are the accepted forms of payments?

We accept payment through Private funds, Social Security, SSI, VA, and Long Term Care Insurance Plans.

What are the required documents for screening?

The pre-screening required documents are the following:

  • Psychosocial summary if applicable
  • Proof of benefit

Prior to admission a Medical evaluation (3122 or 4449) and Mental Health Evaluation (if applicable) need to be completed.

Can a husband and a wife share a room?

Yes, a husband and a wife can share room. Other domestic partners can share a room as well.

Are meals included?

Yes, we provide 3 nutritious meals, and snacks throughout the day. All menus are prepared by our nutritionist/dietitian.

What types of transportation is available?

Upon request, we assist our residents in arranging their transportation to off-site medical appointments. We also provide transportation to various off-site recreational activities in select locations.

Will I be allowed to have visitors?

Yes, all residents are allowed to have visitors and can leave the facility to visit friends and family at any time.
